Please note, by signing up for all EXCEL Basketball Academy programs, you will be asked to consent to both the COVID PROTOCALS and the RISK LIABILITY WAIVER. Please review the information below.
COVID PROTOCOLS

I acknowledge and confirm that my child (EXCEL player) will follow all Covid protocols:
I confirm that EXCEL player does not have a temperature of 100.4 degrees F. or greater.
I confirm that EXCEL player does not have any of the following symptoms: cough, shortness of breath, chills, muscle pain, headache, sore throat or new loss of taste or smell.
I confirm EXCEL player has not within the last 14 days tested positive for COVID-19 or been in close contact with anyone who has tested positive for COVID-19.
I confirm that if any of these conditions occur, EXCEL player will stop all training until they receive a medical release.
PHOTOGRAPHY AND SOCIAL MEDIA RELEASE
This photo release grants permission to EXCEL Basketball Academy, hereinafter known as EXCEL to photograph and take images/videos of my minor child during any basketball training, including but not limited to basketball practices, camps, league games, etc., in and/or for promotional purposes without payment or consideration. This release includes, but is not limited to, using the images/videos on the internet/social media, newsletters, websites, emails, brochures, magazines or other general publications and in whatever manner EXCEL finds useful or for promotional purposes or any other lawful purposes.
I hereby waive any right to inspect or approve the images/videos that may be used now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of any images/videos.
I am the parent or legal guardian of the child I am registering or have already registered to participate with EXCEL. I have read this release before acknowledging it, and I fully understand the contents, meaning and impact of this release. This release shall continue indefinitely, unless I otherwise revoke it in writing. By registering my child for any EXCEL program I understand that I have agreed to this release.
RISK LIABILITY WAIVER

I am aware that EXCEL Basketball Skills Academy (“EXCEL”), will be providing basketball training, and that certain risks and dangers will or may occur, including, but not limited to hazards inherent in basketball, communicable diseases such as Covid-19 virus, negligence or other careless acts and omissions by other participants and dangerous conditions of facilities. The terms hereof shall serve as a release and waiver of claims against EXCEL and its employees and contractors, and assumption of risk for me, and for all members of my family, including any minors accompanying me or participating with EXCEL. I hereby give my permission for my son/daughter to attend the EXCEL training. Further I attest that I am the parent or legal guardian of the participating child and that I have full authority to consent to all of the above. I consent to allow my child to receive emergency medical treatment as deemed necessary and appropriate.
